IWM

Gas Warfare and Medical Treatment in the First World War

The use of chemical weapons added another dimension to the medical challenges of industrial warfare. Gas attacks could affect large numbers of soldiers at once, while some chemical agents produced injuries whose effects developed over time.

Medical personnel had to identify different types of gas injury, provide treatment and evacuate casualties from contaminated areas. At the same time, armies developed protective equipment, warning systems and procedures intended to reduce the number of soldiers exposed.

The Introduction of Gas Warfare

Germany used chlorine gas on a large scale at the Second Battle of Ypres in April 1915. The attack demonstrated the potential of chemical weapons to produce mass casualties and created a new problem for military medical services.

Both sides subsequently developed and used chemical weapons. The nature of gas warfare changed as new agents were introduced, methods of delivery developed and protective measures improved.

Chlorine Gas

Chlorine was one of the first chemical agents to be used extensively. When inhaled, it could cause severe irritation and damage to the respiratory system.

Exposure could produce coughing, difficulty breathing and damage to the lungs. Severe exposure could be fatal, while soldiers who survived could require prolonged medical treatment.

The introduction of chlorine demonstrated how a new weapon could create a medical problem for which existing battlefield treatment was poorly prepared.

Phosgene and Other Gases

Other chemical agents were subsequently developed. Phosgene could cause serious damage to the lungs, while other gases produced different combinations of respiratory, eye and skin injuries.

The symptoms of exposure varied according to the chemical involved, its concentration and the duration of exposure. Medical personnel therefore needed to recognise different forms of injury rather than treating all gas casualties in the same way.

Mustard Gas

Mustard gas was introduced by Germany in 1917 and produced a different pattern of injury. It could damage the skin, eyes and respiratory system, while some symptoms developed only after a period of time.

Mustard gas could cause severe blistering and burns to exposed skin. It could also damage the eyes and lungs, creating casualties who required prolonged medical care.

The delayed effects of some exposures made medical assessment particularly difficult. A soldier who initially appeared relatively well could subsequently develop serious symptoms.

Gas Injuries and Medical Treatment

Gas casualties could suffer from respiratory problems, burns, eye injuries and damage to the skin. Treatment depended upon the type of chemical involved and the severity of exposure.

Removing contaminated clothing and cleaning exposed areas could form part of the immediate response. Soldiers with respiratory injuries could require prolonged observation and treatment, while serious eye and skin injuries needed specialist care.

Gas casualties therefore placed additional demands upon the wider medical evacuation system. Soldiers had to be removed from contaminated areas and transported through medical units capable of providing appropriate treatment.

Medical Services and Evacuation in the First World War

Protective Equipment

The medical response to chemical warfare was not limited to treating casualties. Preventing exposure became increasingly important as armies developed more effective protective equipment.

Early protection included improvised methods, but gas masks and respirators became increasingly sophisticated during the war. Soldiers also had to be trained to recognise gas attacks and respond quickly when warnings were given.

The development of protection illustrates an important feature of wartime medicine: medical knowledge, military technology and preventative measures increasingly worked together.

Gas Warfare and the Front Line

Gas attacks were particularly challenging because chemical weapons could affect soldiers across a relatively wide area. Wind direction, terrain and weather could influence the effectiveness of an attack and the movement of contaminated air.

Gas therefore created a medical problem that could extend beyond individual wounds. Medical units had to prepare for groups of casualties arriving together, while soldiers needed to respond rapidly to warnings and protect themselves from exposure.

The Western Front provides the wider context for the development and use of chemical warfare during the war.

Gas and Surgery

Some gas casualties required specialist treatment for severe damage to the eyes, skin or respiratory system. Their treatment formed part of the wider development of wartime surgery and medical services.

The experience of treating severe battlefield injuries also contributed to developments in other areas of medicine, although gas injuries created their own distinctive medical problems.

Surgery and Plastic Surgery in the First World War

Gas Warfare and Medical Organisation

The treatment of gas casualties depended upon the same wider network of medical services used for other forms of battlefield injury. First aid, evacuation, casualty clearing stations and hospitals all had roles to play.

The ability to move casualties quickly became particularly important when large numbers of soldiers were affected by an attack. Medical organisation therefore became part of the wider response to chemical warfare.

X-Rays and Medical Imaging and Blood Transfusion provide examples of other technologies that formed part of the broader development of wartime medicine.

The Psychological Effects of Gas

Gas created a distinctive psychological threat because soldiers could be attacked by a danger that was difficult to see and could affect large areas. The possibility of a gas attack added to the fear and uncertainty of life on the front.

The psychological consequences of combat are considered further in Shell Shock and Psychological Trauma in the First World War.

Gas Warfare and the Wider Conflict

Chemical warfare formed part of the wider technological development of the First World War. The conflict encouraged the development of new weapons while forcing medical services to respond to the injuries they produced.

Gas warfare also demonstrated the increasingly industrial nature of the conflict. Scientific knowledge, manufacturing capacity, military organisation and medical expertise became closely connected.

The wider development of weapons and military technology is examined in Development of New Weapons in the First World War.

The Legacy of Gas Warfare

The medical experience of chemical warfare demonstrated the importance of combining treatment with prevention. Protective equipment, early warning systems and procedures for dealing with contaminated casualties became essential parts of military preparation.

Gas warfare also left a lasting impression on soldiers and societies after 1918. The experience contributed to continuing debates about the use of chemical weapons and the medical consequences of modern warfare.

Medicine in the First World War

Gas warfare was one part of the wider medical challenge created by industrial warfare. Medical services also had to respond to severe physical wounds, psychological trauma, blood loss, infectious disease and the problems created by prolonged military service.

This article forms part of the wider Medicine in the First World War section.

Further Reading

Gas Warfare, Wellcome Collection — medical and historical collections relating to chemical warfare, gas injuries and the medical response to gas during the First World War.

First World War Chemical Warfare, Imperial War Museums — photographs, objects and documents relating to gas warfare, protective equipment and the experience of chemical warfare.

Chemical Warfare in the First World War, Science Museum — material on chemical weapons, gas masks and the development of protection against gas.

Medicine on the Western Front, The National Archives — contemporary sources concerning gas warfare and the medical treatment of soldiers during the First World War.

For the wider history of the conflict, return to the First World War hub.

Gas warfare introduced new and severe medical problems during the First World War.
